We are now using the portal to enable patients to complete their forms at home rather than here at the practice. We hope that this will give our receptionist more time to help patients that would rather complete the forms at the surgery and will make checking in at the practice a quicker and efficient process .

What will I be asked to fill in at home ?

• Check and update any contact details
• Medical history
• NHS FP17 form (Patient Declaration)

How does it work ?

You will automatically be sent the forms you will need to complete a couple of days prior to your appointment via email. Once submitted they will transfer to your notes here at the practice.

Check in with reception or by scanning the QR Code at the practice when you arrive at the practice and that’s it!

If we don’t have your email address but you would like to use this service please email on enquiries@lostwithieldental.co.uk with your Full name and date of birth and we can ensure you are set up to get the portal forms sent to you.
