A gentle reminder for our patients that the COVID screening, medical history and NHS forms (if you are being seen as an NHS patient) needed for appointments do need to be completed online within 24 hours before your appointment.
These forms are accessed by clicking on the link on your reminder email. If you are having trouble with them just let us know and we can guide you through what to do.
If you have been seen recently and are returning for another appointment the COVID screening questions will need to be completed again so that we can ensure there are no changes.
If the forms are not completed at home, the practice then needs to conduct follow up calls to complete COVID screening prior to attending the practice. This is to ensure we are keeping all of our patients and staff safe. By our patients completing these forms at home, you are helping us free up staff to assist our patients within the practice and answer your enquiries more quickly.
